This amulet is received from Dethmold before leaving the Kaedweni camp to cross the mist and reach Vergen. This item is unique to Roche's path. On Iorveth's path, the equivalent is Philippa Eilhart's medallion.
While crossing the mist, Geralt can use this amulet the same way he normally uses his medallion (by pressing the 
) and a pinkish floating, glowing bald head with four faces will appear showing him which way to go.
